- Mandatory Information : Registered patient status, patient name, gender, nationality, date of birth, residency, overseas health insurance enrollment status and policy number, e-mail address, mobile phone number

- Optional Information : Patient ID (registration number), address in Korea, overseas address, telephone number, duration of stay

3. Retention and Use Period of Personal Information

- In accordance with the Medical Service Act

You may refuse to consent to the collection and use of personal information. However, refusal may result in restricted access to services.
